What is ICD-10-CM?
The International Classification of Diseases, 10th Revision, Clinical Modification is the US code set for diagnoses on claims and medical records. NCHS (part of CDC) maintains it with CMS, adapted from the World Health Organization's ICD-10.
What makes a code billable?
Only the most specific code in a branch is valid on a claim. A header code (such as E11) groups more specific codes and is not billable; pick the code under it that describes the condition fully, including a 7th character where one is required.
What is the difference between Excludes1 and Excludes2?
Excludes1 means the two conditions are never coded together. Excludes2 means the excluded condition is not part of this one, but a patient can have both, so both codes may be reported.
When do the codes change?
Every October 1, at the start of the federal fiscal year, and sometimes on April 1. This lookup is updated from the CDC files for each release.
