CASPer Exam: Transforming Medical School Admissions

Why CASPer Matters

The CASPer exam (Computer-Based Assessment for Sampling Personal Characteristics) has become an increasingly common requirement in medical school admissions. Unlike grade point average (GPA) or the Medical College Admission Test (MCAT), CASPer assesses noncognitive skills such as empathy, professionalism, and ethical reasoning, reflecting a growing emphasis on holistic admissions.

Evidence to Date

CASPer presents applicants with real-world scenarios requiring short, timed responses in video or written format. Raters score answers based on clarity, fairness, and judgment rather than academic knowledge. Early research suggests CASPer can complement traditional metrics by highlighting interpersonal qualities that predict professionalism in clinical training. Medical schools are adopting the test to gain a broader view of applicants beyond grades and test scores.

Ongoing Debates and Research

While promising, CASPer’s predictive validity is still under study. Questions remain about how well performance correlates with long-term clinical success and whether socioeconomic or cultural factors influence scores. Critics note variability among raters and institutions, while proponents argue it reduces overreliance on standardized tests. Clinical Guidance for Physician-Educators

For clinicians mentoring premedical students, preparation should focus on practicing structured, ethical reasoning under time pressure. Applicants should be encouraged to demonstrate empathy and professionalism consistently rather than seeking “right answers.” Admissions committees should interpret CASPer scores as one part of a holistic process, balancing academic, experiential, and interpersonal domains.
