First messenger RNA influenza vaccine outperforms standard-dose vaccination

1. mFlusiva is the first messenger RNA influenza vaccine approved in the United States, with the FLUENT phase 3 trial demonstrating 26.6% greater relative vaccine efficacy than a licensed standard-dose influenza vaccine in adults aged 50 years and older.

2. The platform may ultimately allow influenza vaccine production closer to the circulating season, but higher reactogenicity and the need for confirmatory evidence in adults aged 65 years and older remain important considerations.

Seasonal influenza vaccination has historically depended on manufacturing platforms that require strain selection months before transmission peaks, creating an inherent risk that circulating viruses will diverge from those represented in the vaccine. On August 5, 2026, the U.S. Food and Drug Administration (FDA) approved mFlusiva (mRNA-1010), making it the first influenza vaccine based on messenger RNA (mRNA) technology. The pivotal FLUENT phase 3 trial randomized more than 40,000 adults aged 50 years and older to mFlusiva or a licensed standard-dose influenza vaccine and demonstrated 26.6% relative efficacy against confirmed influenza-like illness compared with the standard-dose vaccine. Traditional approval applies to adults aged 50 through 64 years, while approval for adults aged 65 years and older was granted through the accelerated pathway and requires confirmatory evidence to verify clinical benefit. The platform is particularly relevant to influenza because messenger RNA manufacturing does not require propagation of vaccine virus in eggs, potentially allowing vaccine production to begin closer to the upcoming influenza season. A shorter production timeline could eventually reduce the interval between strain selection and manufacturing, although the current approval does not establish that mFlusiva will consistently achieve better strain matching or greater effectiveness across future seasons. The trial therefore establishes superior efficacy against a standard-dose comparator during the studied season rather than proving an inherent clinical advantage for messenger RNA technology under every future influenza scenario.

Reactogenicity was higher with mFlusiva than with the standard-dose comparator, particularly for injection-site pain, fatigue, headache, and myalgia, making anticipatory counseling relevant when discussing vaccination. Serious adverse events occurred at similar rates between groups, with few considered related to vaccination, supporting an overall safety profile that must now be characterized across broader postmarketing use. The FDA indication covers prevention of influenza caused by represented influenza A subtypes and influenza B in adults 50 and older. For clinicians caring for adults aged 65 years and older, the distinction between accelerated and traditional approval is important because continued authorization in this population depends on verification of clinical benefit rather than immunogenicity alone. Experience across multiple influenza seasons will also be necessary to determine whether faster manufacturing translates into meaningfully improved antigenic matching when circulating strains evolve after conventional strain-selection decisions have been made. The immediate clinical significance is therefore straightforward: physicians now have an influenza vaccine using a fundamentally different manufacturing platform that demonstrated greater efficacy than a standard-dose comparator in a large randomized trial. Whether mFlusiva ultimately changes influenza prevention more broadly will depend on its performance across seasons, confirmatory evidence in older adults, real-world effectiveness, and whether the theoretical manufacturing advantages of messenger RNA translate into consistently better protection.
